Small Business of the Month Nomination Form
To be filled out by person nominating the business:
Criteria For Selection

Ownership - The applicant
must be an owner or co-owner of the business.

Size - 25 employees or less and whose common stock is not regularly bought and sold in a public securities market except locally owned franchises of publicly held corporations.

Membership - Must be a member of the Lafayette - West Lafayette Chamber of Commerce.

Age - Must have been in
business for (3) three years under current ownership.

Chamber/Community Involvement
- The business and its owner/manager must have a record of
active community involvement.
Rules

Nomination letters may be submitted to The Chamber and The Chamber will contact these businesses directly for application materials. Businesses may nominate themselves.

Information supplied by the applicant will be provided to the Small Business Committee.

All award winners will become eligible for The Lafayette - West Lafayette Chamber of Commerce "Small Business of the Year" Award.

The selection committee reserves the right to make no award.
